If a participant says they do not have a user ID or sign in. Ask them to register for the site using their personal information. The account number is not required.
Once they are signed in on their mobile device. The CAM Instructor will need to submit the form below with the USERs information.
This will deliver the course materials to the USER, using the email address supplied on the form, and send the office a notification to research the billing.
